The thing keeping Tony Stark alive is also killing him, and he tells nobody.

Six months after announcing himself, Stark is a celebrity, a one-man deterrent, and quietly dying: the reactor in his chest is poisoning his blood. He responds by behaving worse, which is a more honest portrait of the character than any speech about responsibility.

Meanwhile a man whose father was written out of Stark family history builds the same technology in a basement in Moscow, and the film’s real argument arrives with him. Stark’s defence has always been that nobody else can do this. Ivan Vanko does it in six weeks with scrap.

More setup lives here than in any other Phase One entry, and some of it is buried: the wider organization behind Nick Fury, a second armored character, a government that wants the suits, and a briefcase of material that becomes important two films later.

If the middle feels crowded, that is why. It is doing two jobs.
